To apply for a personal licence, you must complete the following forms and obtain the following information:

• Application for a Personal Licence Form
• Disclosure of Convictions and Declaration Form

The application forms are available to download from the related pages section on this page. Both forms are obtainable through the one download.

Personal licence holders pay an initial application fee of £37.

Your application must include two photographs. The photographs must:

• be taken against a light background so that your features are distinguishable and contrast against the background
• measure 45 millimetres by 35 millimetres
• be full face, uncovered, and without sunglasses
• be without any head coverings, unless worn for religious reasons
• be on photographic paper
• be certified as being a true likeness by a solicitor, notary, individual with a professional qualification (such as chemist or accountant), bank or building society official, police officer, civil servant or minister of religion.

As well as the application form, fee and your photographs, you must also send in:

• your approved licensing qualification certificate (further information under related items) - this must be an original document - and
• either a criminal conviction certificate (if you have been convicted of criminal offences) or a criminal record certificate from the Criminal Records Bureau or a subject access search of the police national computer by the National Intelligence Service (further information under related items)

Your completed application must be submitted to the Town Hall, either by hand or by post, for the attention of the licensing team. Your application will be processed and you will be contacted if there are any queries.

Unless there is a need to register a change of name or address, or to obtain a copy of a licence, there will be no further fees to pay until the personal licence is renewed. The fee payable for updating a personal licence or to request a copy licence is £10.50.
